We are happy to complete all medical reports at the request of a solicitor or insurance company. Please drop any forms into our reception team (or email to office@athboyfamilypractice.ie). We will then contact you regarding any necessary consultations and/or fees. Fees may differ depending on the report requested. Payment is required prior to release of the report.
